Responsibilities: Provides inpatient care coordination services for medical/surgical patients to facilitate optimal patient outcomes, support the delivery of effective and efficient care, improve continuity of care and efficient use of resources throughout the episode of care. The case manager will collaborate with the Care Team including, but not limited to: Attending/Specialty Physician(s), Social Work, Utilization Review, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with the smooth transitions in levels of care.

Qualifications:

Associate Degree of Nursing with RN Licensure from an accredited College or University; Bachelors or Masters Degree of Nursing preferred. Minimum 3 years experience as a Registered Nurse in a clinical setting, performing utilization review, case management or discharge planning, care and/or quality improvement. Current State of Minnesota Nursing License

Additional Information:

Must be able to work at least 1 weekend per mos & flexibility to pick up occassional weekdays preferred. Experience working with EPIC preferred.

Responsibilities:
