Coordinates quality improvement activities through multidisciplinary team recommending, implementing and tracking improvement initiatives. Maintains the Stroke Registry database and reports pertinent patient data timely. Responsible for overseeing the continuum of care of the Stroke program. This includes collaborating with the health care team to assess, develop care plan, educate, monitor, and evaluate the progress of patient care. Acts as a clinical resource to the staff in providing in-services, validating stroke competencies, and advising management of training needs. Ensures educational outcomes are met through coordination and delivery of internal and external stroke educational programs for all professionals and community.  Collaborates with area healthcare providers, referring hospitals/clinicians, the regional EMS and aeromedical services to facilitate patient access to stroke services throughout the continuum.<br/><br/><b>Qualifications:</b><br><br>  Completion of accredited associate / bachelor?s degree in nursing.<br/>Master?s degree in nursing preferred.<br/>Five (5) years clinical nursing experience in a hospital setting to include at least:<br/>Two (2) years in a critical care area and/or emergency setting.<br/>Two (2) years providing nursing care to injured patients.<br/>Neuroscience nursing experience preferred.<br/>Experience in program planning, management and supervision preferred.<br/><br/>Current MN RN licensure<br/>Current ACLS (or equivalent competency) Verification<br/>Specialty certification (e.g. CCRN or CNRN) preferred<br/><br/>Continuing Education requirements:<br/>?         Annually attends at least eight hours of stroke related education.<br/>?         Regularly updates self to content of journals related to stroke care.]]></description>

The NP-C engages in independent decision-making about health care needs and provides health care services to individuals and their families. The NP-C performs a comprehensive history, including chart review and information gathered from the family and performs physical and psychological health assessments.  The NP-C provides direct patient care as well as consultative services to assure state of the art Palliative Care.  The NP-C balances the workload and resources of the Palliative Care team.<br/><br/><b>Qualifications:</b><br><br>  Masters in Nursing or Public Health.  Minimum experience 3-5 years in a clinical setting such as inpatient, hospice/homecare.  Prefer 3 or more years of experience in Palliative Care or related fields.  1-3 years as NP-C.  Current Minnesota RN license.  NP certification in relevant specialty through the ANCC.  Approved by the Minnesota Board of Nursing for prescription writing privileges.
