Job Description:

The case manager will collaborate with the Care Team including, but not limited to: Attending/Specialty Physician(s), Social Work, Utilization Review, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with the smooth transitions in levels of care.

Qualifications:

Associate Degree of Nursing with RN Licensure from an accredited College or University; Bachelors or Masters Degree of Nursing preferred. Minimum 3 years experience as a Registered Nurse in a clinical setting, performing utilization review, case management or discharge planning, care and/or quality improvement. Current State of Minnesota Nursing License

Additional Information:
