Job Description:

Responsibilities: The Certified Nurse Practitioner (NP-C) works in collaboration with physicians and other medical staff to provide Palliative Care services to patients and their families. The NP-C engages in independent decision-making about health care needs and provides health care services to individuals and their families. The NP-C performs a comprehensive history, including chart review and information gathered from the family and performs physical and psychological health assessments. The NP-C provides direct patient care as well as consultative services to assure state of the art Palliative Care. The NP-C balances the workload and resources of the Palliative Care team.

Qualifications:

Masters in Nursing or Public Health. Minimum experience 3-5 years in a clinical setting such as inpatient, hospice/homecare. Prefer 3 or more years of experience in Palliative Care or related fields. 1-3 years as NP-C. Current Minnesota RN license. NP certification in relevant specialty through the ANCC. Approved by the Minnesota Board of Nursing for prescription writing privileges.
